Just want to share that after calling 14 Civil Surgeons in the Houston area that I found 1 that will transfer the vaccination information from the DS-3025 to the I-693A without a complete medical examination. Everyone else insists that they do the complete physical AND that the physical is a requirement. Doctor specifics are:

The receptionist is very nice and new exactly what I was asking. You will need an appointment but they can do this in 15 minutes for $45.
