DS-3025 Medical Examination Checklist

Hi all,

So I'm about to file I-485 and was wondering how we can obtain the DS-3025 checklist since my fiance sumbitted this checklist (seal brown envelope) at the airport. Any thought?

That form was not in the sealed packet. She should still have the 3025 in the larger IMO envelope along with the chest x-ray disc and the other small paper with her photo stapled to it and IMO red ink stamp

No..I-693 was completed in Vietnam and submitted in the sealed envelope when she went through POE.

If you coming from a K1 to i-485 then you don't need to submit a I-693 document. The government already has that information. I know because we had our interview 2 weeks ago and pass.

Reference:

K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. If you are missing any shots from your overseas medical a I-693 vaccination transcription is required to be completed by seeing a Civil Surgeon to have certain portions of the I-693 completed and have the Civil Surgeon certify the form (sealing the original in an envelope and giving you a copy).

05/11/2014 - Bring my fiancée back to the US

Hi guys,

my fiance (now wife) didn't get her vax @ IOM cause they put her off saying unnecessary. Every civil surgeon here i called wants to do a full workup $500+ or more.

my Finance had a packet from IOM she brought over and all that was in it was big envelopes that only had a single CD w/ Xray on it and a small invoice saying medical was paid for?

Do I just need a civil surgeon to do vaccinations/immunization only on a i693 for k1 visa?

For AOS, Is there any other proof/documentation/evidence I need for the medical or do they know that from the earlier K1 process already?

Wow that's a rip off. We paid $50 for a civil surgeon to give him a shot in Seattle (I don't' remember which shot my fiance got). The surgeon filled out our I-693 form, sealed it and gave it back to us. You also get a copy of the form. It was a quick and easy process. Hoping someone in your area can chime in and help.
