Vaccination documents, where & when & how?

I got married to my fiance almost a week ago and we are getting our AOS, EAD forms together now. I was wondering if someone coukd help me out with information on getting the vaccination/medical letter from a doctor, I have no idea where to go for it. I had a mediacl done at home in Ireland before my visa was approved and I saw that I only need a vaccination document or something on the AOS forms.

If anyone can help me it would be really appreciated.

Also does it say on the Advanced Parole where you intend to go, what if you go somewhere different to whats you put on the form? are they strict on that?

You will need to get a Vaccination Supplement filled out by a civil surgeon here in the U.S. Call around to the civil surgeons in your area to find one that will not require you to have another medical exam (as long as your K-1 medical exam was less than one year ago). Tell them you just need the Vaccination Supplement filled out, without another medical exam.

If you got a copy of the Vaccination Worksheet from your K-1 medical exam, take that with you and the civil surgeon will transfer the information onto the Supplement. If you do not have a copy of the worksheet, you can call the office where you had your medical exam and request the worksheet be sent to you.

For AP, once it is approved, it is good for one year, for multiple entries, and you may travel anywhere you want.

Sept. 13/07... N-400 sent to NSC

Aug. 21/08... Interview - PASSED!!!!

Sept. 2/08... Oath Ceremony

Sept. 5/08... Sent in Voter Registration Card

Sept. 9/08... SSA office to change status to "U.S. citizen"

Oct. 8/08... Applied in person for U.S. Passport

Oct. 22/08... U.S. Passport received

DONE!!! DONE!!! DONE!!! DONE!!!

KAELY (K-2 Visa)

Apr. 6/05... DS-230, Part I faxed to Vancouver Consulate

May 26/05... K-2 Interview - APPROVED!!!!

Sept. 5/05... Entered U.S.

Sept. 7/05... I-485 & I-131 sent to CLB

Feb. 22/06... AOS Interview - APPROVED!!!!! (Passport NOT stamped)

Dec. 4/07... I-751 sent to NSC

May 23/08... 10-Yr. PR Card arrives in the mail

Mar. 22/11.... N-400 sent to AZ

June 27/11..... Interview - PASSED!!!

July 12/11..... Oath Ceremony
