I-693 even if we are not yet married

Hello everyone

I just got here in the US last sunday. We plan to have my vaccination record transferred/transcribed this weekend.

Since we are not yet married, i will be filling out the form using my last name.

We plan to get married in March.

Will there be a problem when we pass the I-693 form for AOS and i have indicated my current last name and not the married one?

If your medical examination in St. Luke is still valid, no need to file for I-693. Well anyways back to ur question, yes use ONLY ur single/birthname since you are not yet married.

As you were told in your other thread about vaccinations, you do not need to see a civil surgeon if you have all the required vaccinations and they are marked off on a correctly filled out DS-3015 from your K-1 medical. I did get my missing shots and i-693 vaccine history Page prior to the marriage too, had no problems.

It is not where I breathe but where I love that I live.
