Hello! I just got here in Florida last Sunday and starting to fix some documents (SSN/marriage license etc).

I'm a bit concen about the vaccinations. I completed mine in the Philippines. Do i still need to look for a civil surgeon here to transcribe? Do i need additional vaccines?

When i got here, i handed my medical/vaccination envelope to the officer along with the yellow packet. Will i send another original copy to them when we pass our AOS application? I only have 1 left which is my personal copy

• Make one copy of your DS-3025. Send that copy along with your AoS package
• You don't need to transcribe DS-3025 to an I-693 as long as the box - completed- is ticked or you get blanket waiver for incomplete history
• You don't need to complete whole series of your vaccinations. The first series would be fine

What if I am a K nonimmigrant visa holder and already had a medical exam overseas?

If you were admitted as a:

a. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e); or as a

b. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen; and

c. You received a medical examination prior to admission, then:

(1) You are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination, and the medical examination did not reveal a Class A medical condition, or if you did have a Class A medical condition, you received a waiver of inadmissibility and have complied with the terms and conditions on the waiver.

(2) Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit the vaccination record and page 1 of Form I-693.

I had a long discussion about this on VJ site months ago, and got some good advice, particularly from Nich-Nick.

So to make the long story short, we didn't go to civil surgeon and we didn't submit DS-3025 or I-693.

Apparently, if your medical packet that you turn in during POE makes it back to USCIS and

its completed properly, then no civil surgeon prescribing is needed.

Yes, I'm taking my chances, but if they require us to do so,

then we will do it at that time. Apparently, some folks have been successful doing this.

Hi, I have a complete vaccination too but my medical is class B (latent TB). Do I need a civil surgeon for it? Sorry if I hi jack this post, I could not post on VJ and I wonder why. Could only comment but not post a new thread.

We did not go to a CS...we sent the DS-3025 along with the instructions from the website (what Girl from Celebes posted). We had no problems at all. GC in hand ready to file ROC in August.

All you need to do is send in a copy of your DS-3025. That's it, and it is exactly what we did when we sent our AOS application in.

Follow up question:

We got married today and we will receive our marriage certificate in 2-3 weeks.

But i already have a doctors appointment next week for my I-693.

Can i use my married last name in the form even if i still don't have the marriage certificate?
