After preparing the Affidavit of Support bill from NVC using James shortcuts, how is the single sheet with barcode utilized?

Is it folded and inserted with the bill and money order? Or should it be left flat and mailed in a large envelop?

You need to cut the Barcode (payment slip) at the dotted line. Use a paperclip and attach the check and the payment slip together into an envelope. I then put the envelope into a USPS overnight express envelope and sent it off.

I wrote on top of the envlope the NVC case number with my wife's Name and DOB as well as my own.

Good luck.

There are two sheets.

One with the "billing" -

The other sheet just has a barcode near the top.

If you download the form from the James' shortcuts, it should only be one sheet, mostly blank with a payment slip at the bottom which has the barcode at the bottom.

Apparently the form has been modified. The solution: go to the first character and backspace = this will "move" the barcode UP to the first page where it will appear under the billing statement where it belongs.
