Ok My fiancée, now my wife :dancing: was not given a copy of the 3025 from the doctor. In her interview the reviewer at the consulate asked if she received a copy and she told her no and was never given a copy. But the Case officer at the Consulate reviewed everything a said all was in order.

Now we are applying for AOS,

Question is, has anyone here applied for AOS without a 3025 or proof of Immunizations???

Are we to assume that the medical packet the consulate had and forwarded to US has all of that in there??

What if I don't have a DS-3025?

If you are positive that you got the needed shots, then actually mailing in a DS-3025 photocopy is not necessary. The original form is with your other medical results and was turned over to USCIS at POE. That's actually the one that counts because it remained in the "chain of custody" so you didn't have an opportunity to alter it. The photocopy isn't really an official copy and is more for a reminder that you got complete at your visa exam. Of course without having a copy, you don't have the opportunity to see if the rest of the form was completed correctly.

Oh same here! :( I did not receive DS 3025 too but will still go ahead and file my AOS now to avoid further delays. I just wrote an explanation as suggested by KayDeeCee and added some supporting documents like my vaccination booklet and lab results from my medical exam. I hope that will work!
