I don't have an NVC cover sheet to submit documents

We're at that stage where we have to fill the IV form online, and send the supporting documents and AOS to the NVC. The IV and AOS fees are paid. My document package is almost ready to be sent. Online instructions say

mail your financial forms and evidence, supporting documents, translations, and photocopies, with the Document Cover Sheet included with your NVC Welcome Letter, to the following address:

National Visa Center
Attn: DR
31 Rochester Ave. Suite 100
Portsmouth, NH 03801-2914

What's this cover sheet ? Maybe I lost my cover sheet, maybe I never got it. I really don't remember getting one. Anway, I don't want my case to be delayed so how can I get it now?

Go back to the website where you paid the fees and follow the directions to print out the cover sheets. The sheets are usually available 3 days after paying.

Oh, thanks! I did what you said and got the two cover sheets. Do I need to send in both of them? The language of NVC seems to suggest I send in only one: "mail your financial forms and evidence, supporting documents, translations, and photocopies, with the Document Cover Sheet included with your NVC Welcome Letter, to the following address:"

And by the way, why does it talk about the cover sheet that came with the welcome letter? Are the ones I'm printing from the payment portal the same thing?

It was my understanding that the two fee receipts are to be taken to the interview. The Document Cover Sheet it is asking for is page 2 of 3 in the welcome letter. It lists everything you need to include and has checkboxes.

I don't remember everything precisely but I believe there are two different cover sheets. One is specific for AOS. There is bar code on it isn't there? Check the labeling of the bar code itself.

Please help, I'm still confused. Firstly, I don't have any coversheet from my welcome letter- maybe I lost it. Secondly , I did manage to print two coversheets from the payment website- they both have barcodes- one is for AOS application and one is for the IV application. When I send stuff to NVC, the current instructions are that I should send the supporting documents for the IV application and the AOS with supporting documents all in ONE envelope. So which coversheet should I use? Only one? Or both? And how do you know-- because the instructions say use the coversheet from the welcome letter. Please tell me based on your experience of what works, if possible. Thank you!

I think that's how NVC is doing it now, sending the AOS and IV packets together. I'm sorry I'm not more help, but all I can tell you is that with my welcome email there was a barcoded Document Cover Sheet with our case number on it. This included a list of what they want me to send, and boxes to check off either Enclosed, Unavailable, or Not Applicable. On that list it did not include anything about the barcoded sheets for the fees. I assume this is because it gets marked as "paid" in their system as well and they don't need to see the receipts. You can check the website for more info though.

What you wrote in the first post here is the exact same thing my welcome email said too, and I got the Document Cover Sheet. I would suggest calling NVC and asking them directly how they want you to proceed.

I see... well as I said my "welcome letter" was back in 2011 and I didn't get a cover sheet.

Oh...I was reading through my cell phone and didn¸t see this. Hmm, AOS and IV cover sheet have barcode on it. Maybe you can write your own "cover letter" for each package and enclose this.

But call NVC and ask what to do. Maybe they can send you new "document cover sheet" for your case!

Just use either the AOS or IV barcode cover sheet. They have the same barcode.
