Hi folks,

Sorry this is such a repetitive subject but I didn't find a thread that quite addressed my worry....

My medical package consisted of the x-ray, the sealed envelope, and two separate papers regarding my x-ray and vaccination which were mine to keep. However the vaccination record didn't have any "I-693a" or "DS-2035" on it anywhere and it says "for use in Canada only".

I'm (acceptably) healthy, and all my vaccinations were complete before entering the US.

Sooo. Any other Canadians been there done that? Do I need to contact my doctor again for a different form? Eh??

Oct 16 2006 - Finally received the RFE that's been holding up our case. It's the IMBRA RFE that we already sent in and was verified received!

Oct 17 2006 - Senator Mark Dayton's office finally gets USCIS to pull out petition for special processing.

Oct 24 2006 - FINALLY APPROVED!

Jan 18 2007 - Interview in Vancouver / K-1 received

Jan 23 2007 - I enter US

Jan 26 2007 - civil wedding

Feb 17 2007 - ceremonial wedding!

I used the "for Canada only" Vaccination worksheet and got it transfered to the I-963a with no problems. I didn't even think to check that it didn't say it was the DS - 3025 - I just gave it to the civil surgeon here. You should have no problems whats so ever getting that changed. But look around for a surgeon that will just transfer that document - NOT give you the entire medical again because you don't need it! Some people here get ripped off by the surgeons!

My hubby is from Canada and we received the vaccination supplement. I didn't know about a civil surgeon transferring info to I-693 until after my AOS was submitted. We sent the vaccination supplement. We did not receive a rfe for the vaccination supplement. I don't reccommend not following directions but you might save a few bucks. I read of other vj's from Canada making the same mistake. I don't know if those individuals received rfe's but i did not!

My husband is from Canada. We went to a civil surgeon here and got it transferred onto the correct form for $25.00. Just make sure that you don't let the civil surgeon here tell you need a medical exam because you don't if you had it within the past year. In my husband's case, he was missing a vaccination and my family doctor gave him the shot and then went back to the place so everything could get transferred.

Once we had it transferred onto the correct form, you will get a sealed envelope to go with your AOS and a copy for yourself.

My husband is from Canada and did his K3 medical in October. For the AOS package, we submitted the vaccination supplement only (no form), and mentioned in the cover letter that his medical results were submitted at the POE. Will we get an RFE? I'll let you know.

What you (and I) received is a copy of the vaccination worksheet. Some VJ'ers (as have posted their experiences here) had the information transferred to the vaccination supplement form and sent it. I sent in a copy of the worksheet to prove that I had my vaccinations and also to prove the date of my medical (which fell well within the year guidelines) -- I had no problems. Other VJ'ers did the same as I did and received an RFE for the supplement/repeat medical. Others, not wanting to risk it, have gone ahead and gotten the supplement. If you do decide to get the supplement, follow the excellent advice here in this thread -- you do NOT need another medical (if it's been less than a year since the last one) and shop around for the best deal, if possible.
