So my fiance recently got his K-1 medical examination. The doctor requested a blanket waiver for a lot of the vaccinations, and on the Vaccination Documentation Worksheet it says "US vaccination requirements complete, requesting blanket waiver".

However the panel physician then handed him a note to take to his GP. It says that he either needs the records or the booster shots listed on the note (minor vaccinations, like flu shots, mumps and measles, chicken pox etc).

We're a little confused, since the official form states that we've completed the requirements but the unofficial note seems to hint that we need extra boosters and records.

Any insight?

Specifically list what it says on his sheet (either dates or columns to the right ticked) beside

Tdap, Td, DTP (the tetanus, Diptheria, pertussis variations)

MMR (mumps, measles, rubella)

Varicella

Influenza

The rest should be ticked "not age appropriate".

Then I could better answer you.

The rest say Not Age Appropriate or Not Routinely Available. Then there is the hand-written note on plain paper he received:

Dear Doctor/Nurse,

The above named patient currently registered in your care is applying for immigration in the United States. Under the US immigration and nationality act for immigration to America, they need to have the below vaccinations as part of their medical requirements for entry into the country.

The applicant has chosen to attend your surgery for administration of the following:

MMR x 2 If one dose has been given then please give second dose or proof of immunity to measles, mumps and rubella.

Proof Tetanus, Diphtheria and Polio Vaccine given in the last 10 years or booster.

Proof given 2014/2015 Seasonal Flu Vaccine.

Ok, he needs Tdap, MMR, and a flu jab before adjusting status because there are no dates for when he had those shots. VH means he had chickenpox and gets a waiver for varicella. He can get them at his GP possibly for free. But he will still need to pay a civil surgeon in the US to sign off on an official immigration form I-693. He will use his GP proof of immunization to show the civil surgeon, who will copy the shots and dates to the new form. (And charge him a lot of money and probably tell him he needs a new medical exam.)

Had he gone to the GP for shots prior to the appointment at Knightsbridge, then the Knightsbridge form (DS-3025) could have been signed off and used for AOS.

If the medical exam was recent, today or yesterday, it is possible his results have not been couriered to the embassy yet. He could call Knightsbridge and ask them to hold his results until his GP faxes proof of the shots. Then they could re-do the vaccination form showing the three shots. My husband did that and Knightsbridge mailed him a revised form DS-3025.

MMR is two shots, but if he got one and four weeks had not passed to safely get the second dose, then his Knightsbridge form would be marked in the column "insufficient time interval". That is an excuse for a blanket waiver. He is also given a waiver for "not age appropriate" shots.

So back to MMR if he doesn't get it corrected at Knightsbridge propr to them sending it off to the embassy-- he gets one at the GP. He doesn't have to wait four weeks for dose 2 if he is ready to move. But four weeks would have passed by the time he sees a civil surgeon who would say he was due the second dose before he would sign off. Enough time has passed that there is no "insufficient time interval" waiver. So if he hasn't gotten an interview, visa, and left the UK before his four week MMR interval, then get it for free before he comes to show the civil surgeon.

Wouldn't it be easier to put a completed DS-3025 in you AOS packet and not have to go through these experiences?

I completely agree, we'd love to get these vaccinations done far before he comes over. I just need some clarification on one thing:

Does he HAVE to get the DS-3025 corrected before they courier it, or can he just go to his GP and get the proof to take to the interview? He lives quite a distance from Knightsbridge ad the journey would be financially taxing on us both, so being able to simply get the vaccinations locally would be easier if we can do it.

A K1 can get a visa with zero shots. They will not care at his interview if he went to the GP or not. Total non-issue.

If he got the DS-3025 corrected before it went to the embassy, then an acceptable clearance for all medical would be in his file. It ends up in the US and gets matched up with his AOS application file. Once it gets couriered to the embassy 3-4 days after exam, then the window has closed and he will do the civil surgeon route in the US. You can't get it back to fix once it leaves Knghtsbridge.

He does not have to return to Knightsbridge. He would have to call early Monday and ask if they will hold his file until his local GP can fax them the proof of getting the shots. He can get the shots anywhere...even Boots or Sainsbury's, but Knightsbridge needs to see the proof to put it on the DS-3025. If they say "you don't need the shots", he could explain he wants them done and in his medical file. Somebody at Knightsbridge has even told people they can use the GP record for their AOS. NOT true. USCIS will only accept a vaccination form signed off by an approved panel physician (abroad) or civil surgeon (in the US). Knightsbridge does not know the details of the AOS process based on things I have read that they tell people.

(Also, out of curiosity, why did they put "K-1" under "Date"?)

I guess to indicate he isn't required to have the shots for his visa interview. Edited by Nich-Nick

Thanks so much for the help, I don't know what we would have done without this explanation. It's been very insightful and hopefully we won't run into the problems mentioned above. I'll post an resolution update once I'm sure we're past this portion of the journey.
