CFO requirement forms and docs

Hi, first of all, thank you very much for reading this. Im still a little confused if I got everything for my cfo. Please share your experiences/advice if there are more docs that i need to gather. Thank you. The followings are what i think i should get.

  1. Two (2) valid identification cards (IDs) with photograph and photocopy; - NBI and Passport
  2. Original and photocopy of Passport;
  3. Original and photocopy of visa
  4. One (1) 2x2 or passport-size photograph
  5. Duly completed guidance and counseling form; - Where do I get this?
  6. Original CFO guidance and counseling certificate;
  7. Duly completed emigrant registration form; - attached
  8. Originals & photocopy of CENOMAR - No marriage
  9. Payment of P400.00 counseling fee.

Extras
1.The packet that brought to the Interview -

2. Copy of IRS transcripts or tax form with black-out your fiance's SSN - petitioner never marry
3. Photo copy of fiance naturalization certificate
4. Photo copy of fiance passport
5. consent guidance form, need parent to sign - beneficial is 25yrs old

10/02/14 - NOA 2

10/16/14 - NVC received case

10/17/14 - Case # assigned

10/21/14 - Case left NVC to embassy

10/23/14 - Consulate Received

11/17/14 - 1st day Medical Exam at St. Luke's Clinic

11/18/14 - 2nd day Medical Examt at St. Luke's Clinic

11/24/14 - Interview Date - Approved

12/04/14 - Visa Received

01/29/14 - US Entry

Hanks is referring to his website in his signature (here: http://visaconnection-philippines.com/). As for the "Duly completed guidance and counseling form", I believe you have to fill that out while you're there. I think that's the form that Hank's site says is filled out on the 3rd floor. The form is available here http://gcp-appointment.cfo.gov.ph/. Fill it out and print it and bring it with you to your appointment. Completing the form online does not constitute a guarantee that you will fill one of the available slots on the day you arrive for your CFO. Be early, have this form and the other items in Hank's link. Be confident and be sure to know your fiancée's mothers maiden name and his landline if he has one. Most don't so just answer with his cell number. They are not going to check. They are just trying to trick you.

If the form does not work try a different browser. If you cannot get it to work then you can fill one out when you arrive at CFO.

Also, it is very important to know your partner especially his history. If your partner had a divorce from the previous partner, you should know what made their marriage not work. Or if the previous wife died, what's the reason of death. Also, for divorced Fiancées make sure to bring divorce certificates.

The form needed for completing the seminar you get at CFO and complete there. The online form is what is needed to get your sticker after you have your certificate.
