For question #13 on where we were married is city and state o.k. or do they want an address also?

For part #5 "Accommodations for Individuals with Disabilities". If we are not requesting any do we leave entire section blank or check no for her and I on questions #1 and #2? There are no children involved with i-751.

Probably just City and State is fine. (That's all we could give them - outdoor wedding by a lake.)

For accommodations, I think there's a example of a filled form on this site you can check (under Guides). From what I remember, I checked no to those questions and left the children section blank.
