Hey! This is what I sent last week. I am hoping I got it all right and that I won't get an RFE. There may be more that you need to include due to your child. I am unsure on that one.
I put it all in an envelope and stuck labels on everything with numbers on them, like an index.
Also, on the back of the photos I wrote my name and alien number. Here is a good thread to help you out
You will also need a letter to explain how you qualify for AP which is included underneath and can be found here
Hope this helps!!

If any one notices any mistakes at all, PLEASE don't hesitate to correct me. I am not a professional immigration lawyer, I just did a lot of research while waiting for my marriage certificate!! Haha

When I submitted AOS packages, I divided the package in three, so I sent 3 packages in ONE envelope.

1 package for I-485 Application to Register Permanent Residence or Adjust Status,

1 package for I-765 Application for Employment Authorization
1 Package for I-131 Application for travel document
Every package with the corresponding papers for each application, I put a paper clip in each one, so they were visible divided. I didn't mix any document together, so I sent one copy of our marriage certificate for each form (so three in total)

The following thread helped me a lot

You will also need a letter to explain how you qualify for AP which is included underneath and can be found here
is this really necessary?

I didn't include this letter, and I got i-131 approved without any problems. I think you may include it if you know specifically your dates of your trip.

No, this is not necessary or required. No reason is ever needed for AP on the form, and no supplement is needed to give one either. http://www.visajourney.com/forums/topic/507427-info-on-i-131-travel-docadvance-parole-while-aos/?p=7151889

Weird. In the form it says something like 'include a page stating why you qualify for Advance Parole....'

I included it so that they didn't send me an RFE or disqualify me for not doing what the form asked. I'm sure it doesn't hurt.

The form is used for other purposes besides AP and for others besides adjustment applicants. No explanation or reason is needed when filing the I-131 with a pending I-485 as an adjustment applicant. It states this in the instructions on page 8.

Of course they won't deny you for sending it. I was correcting your misinformation in this thread. You told the OP they needed to include an explanation sheet, and caused confusion to others about that. It is not needed. You said it was weird. I pointed out why it was not weird. That is all.

Haha. I know :) That's why I said to correct me if I was wrong.

I think I was just trying to reassure myself. It's such a scary process when you're going it alone. I'm sure all will be good though :D x
