No, that was separate and your fiance is filling up the NBI form on her own, so answer remains "NONE" or left blank.

Thanks.. I'll tell my fiance to stop by the embassy tomorrow to pick up another NBI form and fill it out accordingly.

You will be fine. You may want to look up Hardrockangel, she processed through Dubai recently and may be of guidance. She is now in the USA,

So since she already wrote on question 11 (Purpose) Fiance Visa can she just write;

11. PURPOSE: Fiance Visa / Travel To USA

Then on question 25 (Name & address Of Travel / Employment Agency) Leave it blank?

Yes she can put like that,"fiancee visa/travel to USA,dont worry because NBI manila will going to clarify the purpose Of the application from her representative,so they can simply say travel to USA..i just took my NBI last august,what im suggesting is accurate..thanks to Hank

I'm confused what you mean by representative.

The person that your fiancee will give authorization to process her NBI..she will send the NBI form from philippine consulate to her family in philippines..they are the one who will bring the form inNBI manila..

Are you doing this online? http://clearance.nbi.gov.ph/

Purpose = abroad Purpose Detail = travel USA

The rest is fairly straight forward

Yes, it can be applied for online. But to complete it, you'll have to finish at NBI HQ on Taft for the fingerprinting, etc.

Mar 5, 2015 - USEM Interview Appointment - IR-1 Visa Approved! (CEAC Status updated to ISSUED afternoon of interview day)

Mar 10, 2015 - 3pm - Received SMS saying IV Package ready for pickup at 2GO MoA. Picked up at 5pm. (Shipped/Picked Up/In Hand on 3rd Business Day post interview)

Mar 13, 2015 - CFO Manila - Done

Yes that's what she did. Their's an NBI near where her family lives in Cagayan De Oro City so this all worked out well as she already sent the completed form with an authorization letter to her family via TNT priority mail. Edited by AR&CB

Hi what is the common problem with the nbi bc some if having the 221g? What is about with A.k.a? And the record file what type of file on the nbi does the embassy want?

You being married need to have the NBI in your married name with an AKA for your maiden name. And any other names that you have used, as stated in the embassy instructions letter.

The NBI needs to be for TRAVEL ABROAD, as stated in the embassy instructions letter. (green version)

I'm still using my single name but in my status it says married and have my husbands surname, u think I will have a problem with that? I didn't change my last name yet

Then I would tend to do the NBI in your maiden name with an AKA for your married name, just to cover the bases... it would suck to get done with the interview only to get a 221g
