Philippine NBI Form

Anyone have experience with filling one of these out when applying for a K1 visa? I checked all over the forums but I can't seem to find the answer that I am looking for pertaining to a few of the questions on the actual form itself..

My fiancee is filling out the form because she is in Abu Dhabi, UAE and was told to go to the Philippine Embassy to pick it up. One of my questions was going to be what should she write down for "Purpose" but you already answered it. Someone told her Fiance Visa or K1 so she already wrote Fiance Visa, the other question was "Name and Address Of Travel / Employment Agency", I would think that would be where she is going to be living which is with me at my address.....????

name/address of travel agency... blank or as they suggest ------

So "Name and address of travel / employment agency" should be left blank or shall she write down my home address where she will be living when she arrives?

She has to go to philippine embassy in abu dhabi,they will give her application form,she need to bring that application form to UAE police headquarters for finger printing,then she will bring it back to the consulate,after 3 days consulate will send it back to her and after that she needs to send it in the philippines for final processing,she can ask one of her sibling to do it for her...philippine consulate/embassy doesnt process NBI clearance.i will suggest to do it now to avoid any delays just put purpose to travel to USA...in the end NBI will just put travel abroad.

Thanks for your reply. She already has the form and supposed to finish with the processing so she can mail it to the Philippines by the end of this week but those two questions had her and I confused...

So for question 11 (Purpose) she already wrote Fiance Visa

And for question 25 (Name and address of travel / Employment Agency) she didn't write anything in that field yet because not sure what to write.

Simply put "USA" to the purpose,and live it blank the address for travel agency...in the end nbi will just put travel abroad on the purpose question...if she put fiancee visa The nbi in manila will then ask her representative for the purpose thing,simply tell them travel to USA...

Well, is there a travel agent or employer? Nope? Then leave it blank. The purpose and purpose details makes it pretty clear on intent.

So since she already wrote on question 11 (Purpose) Fiance Visa can she just write;

Then on question 25 (Name & address Of Travel / Employment Agency) Leave it blank?

The reason for the purpose is, if you look on the NBI website, you get charged differently based on the purpose

You guys are doing this process on your own right? So no need for travel agency and she's not getting the clearance as part of a work requirement, so it should be left blank or she should write "NONE."

I had an immigration attorney prepare my I-129F but that has nothing to do with question 25 correct?

No, that was separate and your fiance is filling up the NBI form on her own, so answer remains "NONE" or left blank.
