Any idea if we gave to pay the civil surgeon to complete the 693 form or if that's not required. Calling one nearby tomorrow and want to make sure I'm not ripped off.

Yes, you have to pay the civil surgeon. The fee varies from every surgeon. Before I took my physical exam I search on line the nearest civil surgeons in my place and compare their fee.

My fiance got a CD not a form. :/ omg this process is crazy.

The CD is not sent with AOS. It is yours to keep. It is the x-ray that checked for TB.
