hI all! we recently got married last Nov 15,2014.. and Im contacting some civil surgeon around town in Milwaukee and starting to fill up the required forms. What do I have to write down in all forms of it, my married name or name before marriage??

You have to write in all the forms the name you want in your green card if you changed your name then use that one.

When asked maiden name or others names used then you write your name before marriage.

Use your married name in the format acceptable to the Philippines. First name, paternal family name, husband's family name, dropping your maternal family name in the middle position.

If you have a completed vaccination sheet you do not need to see a civil surgeon. This is specific to K-1/2 visas only. But as long as you had all your required vaccinations, and the others are marked as not appropriate, then you're good to go without one. If you're not a 100% sure its complete, then you might want to see a civil surgeon. I know my wife preferred that just to be sure and not delay her green card.

There's little you can say beyond you're a K-1, and need a transcription only. Its up to the civil surgeon to decide what they want to do. There are some who will say you need to get a full physical, when you do not, but they will not do it any other way. The one I took my wife to, wanted her to get her boosters first, so we took her to our medical clinic and got them free on my insurance. I didn't mind because it was probably the only way I was going to get her to a doctor to actually get the boosters You best bet is to shop around to find your best price and the one who just transcribe the vaccinations if that is all you need.
