ok so we have whole package setup perfect but we are stuck on DS-3025 / I-693 ,we lost his copy of medical record of vaccine and the CD of his scan that they gave him before entering US...what happens if we filed without medical report I heard it is not required if its been less then a year but yet what would happen in our case will he get denied or is there any option we have to get a copy of this vaccinations im extremely nervous and dont know what to do... ?

The original DS-3025 is with your other medical documents that were handed over at your POE when entering the US. If you know for sure all the required vaccinations were marked off properly, then just add in a note that all vaccinations were received at the K-1 medical and the DS-3025 vaccination worksheet can be found in your USCIS file.

This does not guarantee they will not ask for an I-693 at some point, but it is a possibility that you will never have to see a civil surgeon for one.

Thank you so much youve made my day,Yes we are 100percent sure they have it on file as per we had to wait almost 2 months for him to get his visa after being approved because some results werent ready during interview and they had to hold visa until they receive the negative tb results ?

Hey we found the vaccination sheet with receipt and also the CD that was handed to him with the rest of test results it has a sticker that says do not open,Do I just send this or do I have to fill up a form for the medical recort im sending?
