We will be starting to fill out the form for AOS. I was reading the guide and am I correct that my wife, who came here on a K! visa will still need a civil surgen to fill out part of the form even though her medical was less than 6 months ago?

If her DS-2035 vaccine sheet was completed and properly filled, she doesn't have to retake another medical exam. Viet civil surgeons are well-know for their English so chances are DS-2035 was not properly filled (my sister's was filled incorrectly by a Viet surgeon when she was in Vietnam, no VH remark and age waiver wrongfully checked.) In that case, you need to see a surgeon who is willing to transfer it to I-693, it is less than 100 bucks FYI.

If DS-2035 was filled correctly, you only need to include a copy of it in your AOS packet. Good luck.

copying information from Nick-Nich regarding the DS-3025:

Do you need an I-693 immunization sheet? Is my DS-3025 filled out properly?

Look at your DS-3025. If it has the ALL of the following items, you don't need an I-693 (Adults 19-49 yrs old).

1. Shots marked with a date (at least one of a series)

MMR (two are required but if you have "insufficient time interval" checked, then you get a waiver)
Td or Tdap or DT or DTP or DtaP (One no longer than 10 years ago, ie.have a booster)
Varicella or VH written by it if you had chickenpox
Influenza (during flu season)** see note below because you are most likely okay without it.

2. Everything else marked not age appropriate, insufficient time interval, not flu season...things that allow a waiver.

3. RESULTS section filled in with "incomplete" and "may be eligible for blanket waiver" ticked or "complete" (but complete is rare).

4. Signed and dated by the doctor

If it's not completed EXACTLY like that then see a civil surgeon for the I-693. If it is complete, send a photocopy of your DS-3025. Almost isn't good enough. It needs to be filled out properly.

You are missing the Tdap and Varicella. You are required to have these shots for AOS. You will need to get these vaccinations, and then see a civil surgeon to fill out an I-693 form.

Edit to add: you will also need the flu shot since it is during flu season.

Dave, the civil surgeon did mess it up. Did your spouse have Varicella when she was young? If yes, it needs to be marked VH. Tdap must be taken. She marked it wrongly 3 section for Tdap, Varicella and Influenza (not flu season instead.) Sorry but it is time to look around for local surgeon who is willing to transfer it to an I-693 for a fee + missing vaccines. Good luck.

Find a reputable civil surgeon I traveled outside my state to find an honest one. Most wanted to do complete physicals at cost of 1100 bucks when all he needed to do was make sure her vaccinations were up to date and xfered to 693. I was charged 26 dollars and any vaccinations she needed were covered 100% by my private insurance
