Use your own discretion about that part. This question also kind of confused me too, but in the end, I ended telling our "story" and it was almost 3 pages long.

Also, my fiance and wanted to get married, discussed it and that was our "engagement". I even noted in our "story" that it's a bit difficult to have conventional and formal "blessings of marriage" and "proposals" because of our obvious distance. My fiance did contact my Dad and asked for my hand in marriage, but that was the most "formal" it got.

Do you mean what date the evidence should be from? If so, I'd say a range of dates spanning your whole relationship, but most definitely those that prove you have seen each other in the last two years.

In theory yes you can submit them all, but I would just choose a few to create a collection spanning the whole length of the relationship. Try and find conversations where you are talking about things which will support that this is a serious and real relationship.
