I-693 - am I gonna be OK?

Exactly my thoughts Nich-Nick, thank you so much for your reply.

He even wrote the date I went to see him as the date of first examination, page 2 is empty other than the very bottom where he wrote 'none' in the remarks section, page 3 is completely filled out with 'none' in all remarks sections and syphilis is ticked, then screening nonreactive is ticked, and everything else that says no class a/b condition...page 4 is completely empty other than my name and a number, and wow, i just saw that he missed hepatitis b, didn't tick it as not age appropriate or anythng :/ And he ticked vaccination record complete :/ Am I going to get an RFE because of hep b? :(

It sounds like the civil surgeon didn't really know his instructions on how to fill out the form when not doing a whole exam. There is a long memo/FAQ to civil surgeons that says he is only responsible for verifying your identity. He doesn't need to know whether you had a medical exam abroad, or when, or if you had any Class A or B conditions. It says the USCIS will know those things and he is just responsible to evaluate the vaccinations.

Most likely your form will be accepted by USCIS, even though he made a mess of it, because he filled out the shot record you needed.

With a very strict officer, yes you could get an RFE. There are so many officers at local offices and at the national level who approve without interview. Some don't read their instructions very well either, so it is impossible to say what your eventual person will say. You might prefer to go back to the civil surgeon office and explain why you fear the form will not be acceptable and want it done properly. Make a list so they all get addressed.

It is Question 57 that basically says he is not responsible for anything other than vaccinations if that's what you ask for. USCIS will deal with you if you actually did require an exam because they have your records.

As far as the results box he checked. Scroll down this page in the technical instructions http://www.cdc.gov/immigrantrefugeehealth/exams/ti/civil/vaccination-civil-technical-instructions.html#i693 and see what it says.

"Applicant may be eligible for blanket waiver(s).

Completion of a vaccine series is notrequired to conclude the medical examination, because such a requirement would require multiple visits to a civil surgeon and could lead to unnecessary delay in the immigration process. If any of the boxes under the "Not Medically Appropriate" heading was checked, the "Applicant may be eligible for blanket waiver(s) as indicated above" box must also be checked. This box will probably always be checked because some vaccines may not be age appropriate for the applicant."

You could print that too since not age appropriate falls under that "not medically appropriate" heading.

Then send him a bill for tutoring him on his job. :rofl:

I know you said he was already annoyed with you and people tend to shy away from arguing with doctors (not you it seems) but tell him your immigration advisor told you it had to be done over. You know...pass it off as some other annoying person made you come back, even though that is fiction.

You're a true life saver! Will deinitely print all this out and go back there on Monday, I won't even call first. It seems they (CS's) don't deal with filling out the I-693 form for k1 visa holders often enough to know how to do their job properly or in case of the CS I saw, he doesn't know how to do it altogether. I almost feel like filling I-693 out myself and taking it to him to show him what it's supposed to look like lol. I'll suggest that he skips part two of page one and just writes 'Vaccination assessment only' instead of writing anything else that he didn't do and can't possibly know or is completely incorrect. I wish my husband could take a few hours off to come with me, oh boy this CS wouldn't want that :)

Did you get the flu shot at your visa medical or since being here. Make sure it's marked.

I got 2 shots, flu and DTaP through my insurance, both free, and they also did titres for MMR and Varicella (also free), antibodies for both are present in my blood so I'm good to go. I didn't have DS-3025 at all because all my vaccination records were destroyed in the 1992 - 1995 war in Bosnia so that part was completely skipped during my medical overseas.

DTaP is marked, he wrote the date it was administered and marked 'complete' for it, he did the same thing for flu, for MMR he wrote the date titre test was done and wrote something that looks like +immune C2.1/ or something like that and for Varicella he just wrote "V.H." in the Complete Series box. Everything else is ticked as not age appropriate but Hepattiis B, which he completely skipped as it's right above Varicella so I guess he just missed it :(

My fiance's DS3025 says : Vaccine history complete for all vaccines, all requirements met.

That means no transcription will be neccessary correct ?

I think in both our cases the 'complete' box was wrongly ticked and it was supposed to be 'incomplete, blanket waiver may be available' but I'm not sure if USCIS will hold it against us because it's a mistake made by a CS/panel physician. There's no way to know for sure, all we can do is wait.

I hope Nich-Nick will reply and clarify it for us :)

I think from all the reading I have done that "complete" would actually mean you got every shot on the list. And if you didn't get some shots because they were not medically appropriate, then the blanket waiver is supposed to be marked. Some reasons for a waiver include things like pregnant, allergies to eggs, allergic reaction to a prior dose, not enough time since a previous dose, and not age appropriate.

BUT it is common with some panel physicians abroad to mark DS-3025 "complete" and those people got their AOS fine with nothing further. So I kind of agree that if a civil surgeon marks it "complete" most adjudicators are not going to argue with that. Of all the civil surgeons filling out forms, you know there are plenty who don't bother reading the details of their technical instructions. At most the adjudicator might glance over it and see the required shots are recorded without getting too technical about the correct use of "complete".

In the end, it's up to your adjudicator and what he says. I wouldn't go to a civil surgeon if that is the only thing you think might be odd about your DS-3025. You can always go later if they make you. An RFE for AOS is not a big deal that changes your life.

In the case of the OP, she has already paid a civil surgeon and has the opportunity to get it corrected if she chooses. Hers has a number of mistakes that has her a little concerned about getting her money's worth.

Waiting to skip ROC and get 10 yr GC due to over 2 year while pending AOS
AOS APPROVED Oct. 4, 2017 * Green card in hand Oct 13, 2017 !!!!!

First K1 denied after 16 month of AP. Refiled. We are a couple since 2009. Not a sprint but a matter of endurance.

Lol, just when I thought there was nothing else wrong with the form, I notice this. Do you think USCIS would accept this? It really looks like it's been resealed. It's so silly to write 'sealed' before you actually seal the envelope. :shocked:

The instructions for Form I-693 state:

Prepare the original of the completed and signed Form I-693 for submission to USCIS. Place the original of the completed and signed Form I-693 and any supporting documentation into an envelope, and then seal the envelope. On the front of the envelope, write in capital letters: "DO NOT OPEN. FOR USCIS USE ONLY." On the back of the envelope, write your initials across the seal where the flap of the envelope and the envelope meet. Seal the entire flap with clear cellophane tape; make sure that the tape, in addition to the flap, also covers your initials. Give the sealed envelope to the applicant.

Soooo my appointment was today, I showed them the unchecked hep b box and also said my legal consultant filled out the form the way it should be filled out and gave it to me to show to the CS, also brought the printed instructions and other things.... so he just copied what I wrote/checked in the 'sample' form to a new form and then sealed it properly - this time he skipped everything examination related and checked the 'blanket waiver available' box :)

Thanks everyone so much for all your help! I can finally submit my forms now :))))

I love that! Good for you going back and getting it right. So many people are afraid to stand up to what they consider people in authority over them. Very tricky with the example filled out by your "advisor".
