I-693 - am I gonna be OK?

Hi everyone

So today I went to see a CS to transcribe my vaccines onto the I-693 form (I called about 30 civil surgeons, only 4 said they wouldn't require a full medical and this one was cheapest - $55.00). When I got there, he tried to convince me that I needed another TB test and a blood test for syphilis and I refused all that, I brought the instructions for I-693 and showed him the part about K1 visa holders not needing any tests if their medical overseas was done within one year of filing I-485.... ...

He kept insisting, I still refused (he got a bit upset) and as he was filling out the I-693, I noticed that he filled out part 2 too, ticking the box that says no class A or B condition, also he wrote stuff on other pages too, even though I showed him the part which says I only need pages 1 and 5.

Also, he ticked the vaccines I didn't need as not age appropriate and below he ticked the box 'vaccination records complete'. Is that OK? Wasn't he supposed to tick the one saying 'blanket waiver available...'?

Are they going to accept this form filled out this way?

I think if the parts that are needed for your visa are filled out, I think it will be okay. Hopefully someone who has had more experience with this will have a more concrete answer.

Thank you for the reply! I'm hoping the same thing - that they are only going to look at pages one and 5.

I was thinking about attaching a sheet to the form explaining what happened when I went to see the CS - do you think that would help?

I've waited so long for all this to be completed to have this guy mess it up for me

If you didn't have any health issues to follow up on, for example TB, then there is nothing to worry about.

The instructions state they are to fill out Page 1, not Part 1, and the vaccinations page, which is Page 5.

As I understood it, from page one part one and three are to be filled out, if the purpose of the form is just to prove a person's vaccination records.

Part 2 of page one (Summary of medical examination) asks about the dates of examinations and Summary of overall findings - what findings if he didn't perform an examination on me? Even the instructions for I-693 say the following:

Part 2, Summary of medical examination. After the medical exam and any follow up (if required) is complete, summarize the results in Part 2 of Form I-693.

That's why I'm confused.

Did you have Varicella when you were young? If yes, he must write down VH for it.

It sounds like the civil surgeon didn't really know his instructions on how to fill out the form when not doing a whole exam. There is a long memo/FAQ to civil surgeons that says he is only responsible for verifying your identity. He doesn't need to know whether you had a medical exam abroad, or when, or if you had any Class A or B conditions. It says the USCIS will know those things and he is just responsible to evaluate the vaccinations.

Most likely your form will be accepted by USCIS, even though he made a mess of it, because he filled out the shot record you needed.
