name on visa is wrong

Hi everyone, my wife finally (after 15 months!) received her immigrant visa on Friday. When we got home, we realized they made a mistake on her name (both on the visa in her passport and on the cover sheet of the visa packet). They put her first name after her family name and used her middle name as her first name:

Family Name: [Last Name] [First Name]

First Name: [Middle Name]

Is it possible to use this visa, or does it need to be corrected before she can cross the border? Has anyone else had this issue? We're pretty astonished they made such a significant blunder - after all, all they had to do is turn back a couple pages to make sure what they wrote was correct.

hello

same thing happened to me when i applied for a tourist visa to UK , i emailed the embassy , and that was the answer :

´´´ To answer your query, as long as all of your names are spelt correctly and that they are all written on your visa vignette there will not be any problems.´´

i dont know if it is the same thing with your visa , just wanted to share my experience , good luck , salam

Your experience with a UK tourist visa is irrelevant to the OP's questions about a US immigrant visa. The OP also states the name on the visa itself was not spelled correctly and was rightly told by me and NLR that it will need to be corrected before departing to the US.
