Alright I got the form ready and filled out. The only questions I have are.

There are 2 pages of the form. Should I print them on 2 seperat pieces of paper or one piece of paper (front and back)?

Also where do I send the form to? We live now in Tennessee. So I assume I would have to send it to Texas service Center in Mesquite TX. Is that correct? My case has not been transfer to any local office it is still at the Missouri center.

I printed it on 2 different papers, but I guess it does not matter at all. It's right, you file it the Service Center where you filed your K1, or the one who is responsible for you in that area. Good luck. I just did the same last week.
