should the vaccination record and part 1 of I 693 form need to be sealed also ?

i found a civil surgeon that is willing to transcribed on my i693 form for my vaccination record, im wondering if it should b sealed as i get it back from surgeon or its okey even if its not? my whole medical overseas is still on sealed envelope thats why im confuse about this i 693 form thanks for giving time to read my question

The transcribed vaccination record must be in a sealed envelope. We also had my wife's transcribed, he gave us one sealed original to be used with her AOS application, and a photocopy for our records.
