I'm filing AOS now and have two questions, should I put my married name on all paperwork or does it matter if I put my maiden name, then I'm having a big issue with the vaccination worksheet, should I get a CS or just submit the DS3025, I don't want an RFE and it seems like everyone can't give me a straight answer.

I assume u have requested ssn with ur maiden name, after u got married u need to go where ur ssn was requested and change ur old name with the new one, after that u can fill the forms for aos with ur actual name

It doesn't matter. Fill out the forms in whatever name you want on your green card and intend to use on future documents.. I kept my own name.. As for the medical situation I'm not too clear on that as I never went the K1 route so I'll wait for someone more experienced to advise.

Lala is right, whichever name you want on your greencard maiden or married name. K1 vaccination requirements you should look on the forums for information. I think you will find the information you are looking for.

If you are changing your name after marriage, then use your married name on all the AOS forms. Put your maiden name where it asks for maiden or other names used. You will be including a copy of your marriage certificate and that acts as your legal name change document. If you want to keep your maiden name, then use it. You do not have to change your name when you marry. The EAD/AP and green card will come with whatever name you use on the forms.

SSN does not control name change. You do not need to have the name changed on the SSN before beginning to use your married name, especially for AOS purposes. SSA workers will often insist on you having at least the EAD in your married name before changing the name on the SSN anyway.

AMENDED: IF you received vaccinations as part of your medical—I just learned that some countries won’t do this!



We did not go a CS and submitted the DS 3025 along with these instructions: Green Card in hand with no RFE!

3. What if I am a K nonimmigrant visa holder and already had a medical exam overseas?



If you were admitted as a:


a. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e); or as a


b. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen; and


c. You received a medical examination prior to admission, then:



(1) You are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination, and the medical examination did not reveal a Class A medical condition, or if you did have a Class A medical condition, you received a waiver of inadmissibility and have complied with the terms and conditions on the waiver.



(2) Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit the vaccination record and page 1 of Form I-693.
