I called several Civil Surgeons in Ga to see how much it would cost to transfer information from my husbands shot record to the 693a form. Are theses people governed by anyone :angry: . I am a nurse and the prices that were quoted were rediculous. I explained what i needed ,that my husband just had a Physical in Nov 2006 recieved immunization then. The prices were from $200.00 not including shots to 25.00. I will be paying the 25.00 but thats alot just to do one piece of paper.

We had ours done for free. The only civil surgeon listed in our area runs a community clinic type thing and they didn't charge us anything for it. We just went in without an appointment and they did it right then. The atmosphere was a little different that I'm used to but was definately worth it. Maybe you should check and see if any of the people in your area work in the same type of thing. Good luck!

Me as well, a clinic that served the Hispanic community, you have to ring around.

I thought I read on here that someone was able to go there and get their record transferred over. I'm not for certain but somehow this rings a bell. It wouldn't hurt just to call. But I think $25 is a pretty good deal considering everything else.

The civil surgeon we saw transferred the information, took her blood pressure and a couple of other minor examinations. His charge was $ 50 which we thought was reasonable to fulfill our obligation--but half of what her whole physical and vaccinations cost in Manila. So, expensive in comparison, but not the $ 200+ figures some have quoted.

Some people (including me) simply submit the form you get after a K-1 medical (form 2030?) and do not receive RFEs.

Very, very risky. The application clearly states that the petitioner submit an I-693a. I would not recommend any other approach.

Neither do I - all I say is that it worked for me. Everyone needs to choose a risk level (s)he is comfortable with.

Oh, and that was form DS-3025, not 2030.

I only submitted the vaccination worksheet with the AOS application intending to bring the supplement to the interview. I paid $50 at the civil surgeon located across from the Immigration office in downtown Atlanta to transfer the information (because I could find that location). I ended up getting approved without an interview so I still have the supplement. What was really ironic is that the civil surgeon's office didn't use the worksheet - they said they couldn't decipher the panel physician's handwriting! They took the information directly off of my immunization records.
