What exactly is the I-693 medical examination form? I am applying before my husband's medical examination expires from Egypt so what exactly do we submit? A copy of his vaccination forms and what else?

Where do I get the electronic I-94 form again?

When we apply for the Green Card we also want the work permit. Which form is for this? I know there is a form for applying for the work permit WHILE the Green Card is being processed but I want to know which form for filing for the Green Card AND Work Permit at the SAME time.

The general forum recommendation when you file for AOS (I-185), you should also file for EAD (I-765) and AP (I-131). There is no additional fee if filed concurrently with the AOS application. See here for details: http://www.uscis.gov/i-485

Thank you very much, that cleared up a lot! BUT, I still have a few questions. On my husband's DS-2053 form under Varicella it says Not Routinely Available. He has not had chicken pox, so does he have to see a civil surgeon now????? Also, it is Influenza season so does he need to also see a civil surgeon for this???? OR can he just get BOTH shots anywhere and they can mark it on his DS-3025 form??? Thank you!

You can always have the shots done and have the civil surgeon transcribe it in a i693 and include it or wait to see if you get an RFE. Depending on how fast they process it, you may not need them.

This sucks. I don't know what to do. I don't want him to get an RFE but I also HATE the hassle of taking him to get the shots then having to see a civil surgeon UGH

You can always think of it this way.... chicken pox gets worse the older you get so the varicella shot will help prevent a childhood disease. Especially since the little germ factories are back in school (I have a 9 yr old and he's been vaccinated since birth). :) As for the flu shot, my husband's cousin who has been here for several years even told my husband to get the shot his first winter here. Since both our guys came from areas where snow and ice are not as likely in the winter, the flu shot will again prevent a possible crappy sickness. And it may also prevent an RFE. ;)

Varicella is a required vaccination for AOS. "Not routinely available" means it was not given at the medical, and you say your husband never had chicken pox. Your husband needs to get the varicella and flu shots, and then needs to see a civil surgeon to transcribe all vaccinations received onto a I-693. You need to send in the I-693 form with the AOS application. Submitting just the DS-3025 will very likely result in an RFE.

Yeah... I never had chickenpox either or the shot and they put me the varicella vaccine here. He will have to get it for AOS
