I’ve been trying to get in touch with a civil surgeon in Phoenix but they all seem to think I need a second, full medical exam, whereas what I’ve read here is that I just need someone to sign the I-639 and check I have the right immunisations. Does that really cost $350 or am I misunderstanding things?

I've hear that about Phoenix many times. When adjusting from a K-1 visa, most people do not need to have their vaccinations transcribed. So you may not need to do this at all. If the required vaccinations were given, or marked as not age appropriate or some other valid reason, then you're good to go by sending a photocopy of the vaccination sheet from the medical.

If they could not give a required vaccination because they were out of it, or some other condition prevented it (like pregnancy) then you'll need to see a civil surgeon and bite the bullet on what they all seem to do in Phoenix.

I’ve had all my required vaccinations and have the form from the medical in London. Do I just need to fill out the bio bit of the DS-639 and attach the vaccination sheet to it?

No I-639 required at all then. Just photocopy your vaccination sheet and send the photocopy as evidence of the vaccinations. See the I-639 instructions Part III section 3. The I-639 is not required for K Visas if the medical was within 1 year and the DS 3025 was properly completed.

I guess given the date you joined, you arrived recently so I'm assuming you're under a year from the medical still.

Excellent, thank you. Yes, I arrived a couple of weeks ago and we got married last Monday.

Does it change anything if I have a chronic condition? I’m on meds for OCD - is the medical still valid for a full year?

There are some communicable diseases which shorten it, like being treated for TB before entry, but not your condition.

You don't need a new medical or to see a civil surgeon for an I-693 as long as you had all the vaccinations at the K-1 medical marked off on a properly filled out DS-3025.
