In the very first question it ask for the family name. Is that my wife's new married last name or do you use her maiden name?

Next question is Part 3 Section B. it ask to list current spouse and all of your children. So that means I should list my daughter which is now stepdaughter to my wife or is it just asking for my wife's children if any? I have only 1 child who is 16yrs. old.

Last question is Part 3 Section A. it ask for nonimmigrant visa number and date visa was issued. Would that be in the passport in the visa section

Family name is your wife's married name. You will write her married name for the first question.

Part 3 Section B, you will list her spouse (which is you) and all of her children. This is asking about your wife's children, not yours.

Part 3 Section A, the nonimmigrant visa number is the number in red at the bottom right of the K-1 visa. The issue date on the K-1 visa is also what you will use.

Family name is your wife's married name. You will write her married name for the first question.

Part 3 Section A, the nonimmigrant visa number is the number in red at the bottom right of the K-1 visa. The issue date on the K-1 visa is also what you will use.

Let me ask you this also. For the AP card Part 3 #1 it ask you about intended date to travel and how long. What did you put there?

Also Part 4 #1a and #1b what did you put there as well?

For part 3 #1 I just put some random date in the future...with a 2 week duration. It doesn't really matter what you put here. I put an actual date and length of time because the form doesn't let you type in UNKNOWN. Alternatively, if you wanted to you can print it out leaving that part blank, and just handwrite unknown.

Part 4 is left blank. The instructions on the bottom of page 2 state: If you are applying for a non-DACA related Advance Parole Document, skip to part 7...

After you answer the part 3 question1, you skip all the way to part 7 and check the box that says "more than one trip". The example form here is a great example to use and follow.

Thanks..Now for part 7 did you use the Word document example form that was on VJ for part 7 for how you qualify for AP?
