A question about the form g-325a

We finish filing all the forms, and I started translating my birth certificate. In the birth certificate it shows my mother's marriage name.

On form g-325a it asks for my mother's maiden name. I assume I have to put her maiden name and not her last name, correct?

So I'm assuming that the difference in names won't be a problem?!?!

Just like some clearance about the subject.
