Assembling package for uscis doubts with medical papers

I tried to find something similar in the forum but it couldn't find this same question.

Yesterday I picked up the results from the doctor here in atlanta. Sealed envelope DONT try to open it etc they insisted as I already knew. So they gave me a copy to keep and they will keep another one in case something gets lost.

While I'm finally assembling my package I put the envelope and then I realize the instructions say " medical examination and vaccination record" so I look on the copies the doctor gave me and I realize that they only put one page of my vaccination record the one translated in English already.

Do I have to behind the medical envelope attach a copy of the originals and the translated ones with the certified translation signature box???

Or with what the doctor put in the envelope is enough?? I'm confused and I don't want to make any mistake

Nov - 22 - 2014 - EAD/AP approved ?? ( 71 days )

Nov - 24 - 2014 - card in production

Dec - 1 - 2014 - card mailed ??

Dec - 3 - 2014 - Combo card received ??

Dec - 15 - 2014 - email received with interview date for Jan 15 2015! ??

07/07/14 (day 41) Case moved to Testing&Interview

10/20/14 (day 146) Interview scheduled for November 25

11/25/14 (day 182) Interview,Approved on a spot

12/01/14 Received Welcome to USA letter

12/03/14 Card was mailed

12/04/15 Received Green Card on mail




08/30/14 Sent I-765

09/05/14 Txt msg & Email received

09/08/14 Received NOA1

10/29/14 EAD Card Production

11/04/14 Got tracking num for EAD

11/05/14 EAD on hands

Ok thank just I needed someone to clear my doubts, just the envelope from the doctor then since it already says all the vaccines and everything is fine.

The civil surgeon should have filled out the vaccinations page of the I-693, which is page 5 I believe. All you need is the I-693 from the civil surgeon in the sealed envelope.
