Hi. Has anybody recently had to go to a civil surgeon to transcribe vaccination record to Form I-693?

I am done with the medical examination and I have also completed all the vaccinations required. Before I was issued my K1 visa, the medical examination and vaccinations were done last February 2014 by the hospital in the Philippines authorized by USCIS and is valid for one year.
The reason why I need my vaccination history completed on the "Vaccination Record" part of Form I-693 is because they only gave me Hepa B and MMR shots and they didn't have available TDAP vaccination during my medical exam and vaccination visit to the hospital. I was instructed to just get the TDAP vaccine here in the US.
I already got the TDAP and the follow up Hepa B and MMR shots at Albany County Department of Health last April 21, 2014.
I have been calling civil surgeons within my area and they all want me to do the full medical. :(

You have to get the I-693 to the point where it's ready for USCIS. That means you may have to spend extra time, effort, and/or money to do so.

My wife was here on a student visa, and had been in country for seven years before we were married. She *still* had to do a physical. I tried to show them that she had the vaccinations through our local HMO, and they said they didn't care...if Dr. X is going to certify the I-693, then Dr. X is going to do what he needs to do for that to happen (and we had to pay for it).

Yehey!

I am so happy.

One doctor replied to my email and she will do the transcription for me for $85 and its just 15 minutes away from home.
