I am having a problem finding a Civil Surgeon in the Portland Oregon area who understands that my wife does not need another physical exam and just needs the I-693a supplement. I am not interested in paying 170 dollars for this unnecessary exam.

Does anyone know of, or have experience with a Civil Surgeon who will not insist on doing an exam and will just fill out my I-693a?

Yes, I totally understand and was frustrated with the system too. I too came on a K1 and phoned all the doctors who do this around Oregon (I'm very close to Portland). Here is the fellow that I went to and he only charged me $5 for the supplement to be transferred since I had the Ds - 3025 document with me from my Canada medical there.

You can phone if you want but you don't need to set up an appt. You just show up on either Thursdays or Fridays with your immunization record/ or immunization worksheet that you got from your own country and he will transfer it onto a I-963A for you right away. He was extremely nice and very helpful and knew almost every language of all the people that were in the waiting room. Go between the hours of 8am-11am and 1pm-4pm (I went at 1pm and didn't have any prob getting it done within the hour).

Also, there is one other doctor that I called that just did the supplement but it was for $35 and you droped off all your documents and picked them up a couple days later - that doc is:

But i didn't go to him, so i don't know any of the details. I would have rather been there and waited in person. I didn't phone anywhere in central or eastern OR because i don't live around there so you can always phone around there too. But DO NOT pay $170 for the supplement - you don't have to get the whole medical done again if it hasn't been a year!

Sorry for it being so drawn out of a response, but it just annoys me about how rediculous these doctors try to scam people.

No Problem, I knew how daunting it was trying to find a doc here! I was almost at the end of my string - I just didn't understand why it was so hard to find one! :wacko::wacko:

I had all my vaccinations done at my regular doctors, and then went to see Dr F. Douglas Day, who charged $50 to check all the forms, transcribe my UK vaccination record, and the ones i had when i arrived in the US, to the I-693 form, then sign it all off for $50. I took all forms in, was asked to come back in 30 mins, and it was ready and waiting for me to collect. Very efficient, i think it was worth spending the money for piece of mind. The receptionist is very helpful!

They also offer a full range of vaccinations, but mostly i found it was cheaper to go to a pharmacy or a family doctors clinic for those.

Hello...this is an information for people who live in Portland and around.

I talked to Dr A. Torres and he told me he had a terrible accident and he is not working to USCIS anymore at least so far.

I got my vaccination report in Salem and the Dr. Howard Tsang only charged $20. It was fast.

09-28-2007 = NOA1, 23 days after their 45-day promise to send it (grrrr).

10-20 & 11-14-2007 = Phoned ImmOffs; "still pending."

12-11-2007 = 180 days; file is "between workstations, may be early Jan."; touches 12/11 & 12/12.

12-18-2007 = Call; file is with Division 9 ofcr. (bckgrnd check); e-prompt to shake it; touch.

12-19-2007 = NOA2 by e-mail & web, dated 12-18-07 (187 days; 201 per VJ); in mail 12/24/07.

01-09-2008 = File from USCIS to NVC, 1-4-08; NVC creates file, 1/15/08; to consulate 1/16/08.

01-23-2008 = Consulate gets file; outdated Packet 4 mailed to fiancee 1/27/08; rec'd 3/3/08.

04-29-2008 = Fiancee's 4-min. consular interview, 8:30 a.m.; much evidence brought but not allowed to be presented (consul: "More proof! Second interview! Bring your fiance!").

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).
