I'm not sure how to answer this question:

Have you ever been found in violation of customs or immigration laws or other federal import laws? * Yes No

Back in may I was given an I-275 form and had withdrawn stamped on my passport. I was trying to return to the usa about a week after returning. There was a family emergency and I was trying to go back on a one way. I was able to withdraw because of lack of ties to my home country of canada.

It says on my form that I was given a 7a1 letter but I only received the I-275. It says on the form in violation of 212 a7aiI but nothing on my passport. Simply a stamp with application withdrawn. How can I answer this for the nexus application?

As well, I'm applying in my maiden name. My passport is in my married name. I don't want it to look sketchy, I simply want to go back to maiden name due to my separation.

It doesn't sound like you were in violation of anything, just that you were denied entry?
