1. Mailing Address ( next to the Start Here corner page 1 ), who's info should I place here? Is it the Beneficiary? or petitioner?

2. Other information: ( under the Mailing address page 1 ), Country of Citizenship, Date of Birth ( is this for the Beneficiary) ?

3. Not sure if there was an A-number assigned but I don't think I've seen one from the NOA or from the email sent by the NVC. Where will I get this?

4. In Part 4, Information on the Sponsor: are the information needed are that of the petitioner?

5. Family Member: Another I-130 was filed by the petitioner ( for the sister of the beneficiary which is also a minor who is the stepdaughter of the petitioner ) Do we have to write her name under the family member since we made a separate I-864 for her?
