Hi there,

Going through the AOS process now.. Just had a quick question. I know we are supposed to file the I-693a (in a sealed envelope) and I had been talking to another member that said he had a blood test and TB test done at the civil surgeon. Rich says he had a blood test in London at the medical. Does he have to have one again at the civil surgeon? I called and they said it was $260 for the paperwork, blood test, and TB test. I just want to make sure we aren't paying an unnecessary fee! He has all his vaccinations. Thank you!

"If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. If you are missing any shots from your overseas medical a I-693 vaccination transcription is required to be completed by seeing a Civil Surgeon to have certain portions of the I-693 completed and have the Civil Surgeon certify the form"

This makes it seem like he doesn't need to have the I-693 filled out since he's had his medical within the year and he's up to date on vaccinations. At the time of his medical in London he was missing two shots but got them before coming to the US and has proof. His form is marked "Incomplete: May be eligible for blanket waiver due to age." Can someone clarify if he needs to go to CS or not? This process sure does have a way of making me feel overly paranoid about every little detail!!

We just sent the copy of his up-to-date vaccines form (3025?). We mailed it out 2 weeks ago, so no RFE yet. ;)

I hope that's all we need!! Doesn't make sense to have two medicals/tests/FEES, they should do it all in one!! I've read some people say you need the I-693 filled out and others say you don't.. Hmmm….

I've read through "Do you need an I-693 immunization sheet? Is my DS-3025 filled out properly?" and was still unsure.. It says if you have "ALL of the following.. Td or Tdap or DT or DTP or DtaP" then you're okay. If you need all of them why wouldn't it say "and" instead of "or"? And I guess my other question is.. does he have to get the TB test and blood test? The man on the phone didn't seem too flexible on that..

There are four possible adult immunizations.

1) MMR (two are required but if you have the column "insufficient time interval" checked, then you get a waiver. Having one shot is often okay but the waiver bing marked is the surest thing.

2) Td or Tdap or DT or DTP or DtaP (One dated no longer than 10 years ago, or have a booster)

All of those combinations are for the same diseases--Diptheria, Tetanus, Pertussis. Each is a slightly different formulation. Td or Tdap are the adult versions. USCIS will accept the baby formulations which are actually stronger. Capital letters mean more potent.

3) Varicella or VH written by it if you had chickenpox

4) Influenza (during flu season)** Oct. 1- Mar 31.

What shots did he get after his medical? It makes a difference. USCIS will not take any proof unless it was signed by a panel physician or civil surgeon on a proper immigration form.

Post what shots and dates his form DS-3025 says or scan the form and post it if you can't interpret the explanation.

He apparently is ok on DTP if one date is since 2004 but what about the others?

I was missing one jab, so had it done before I went for my Medical. TB was done at the Medical and I think you now get it on a disc, I had the print. Not that you will ever need it again.

Thank you for your reply.. I'm having a bit of trouble reading it to be honest.. To be it looks like he is up to date on MMR, but on another sheet they stated they wanted him to get MMR booster and tetanus booster. He brought that paperwork to another doctor and the gave him a tetanus shot and a polio shot..

He had separate shots for MMR--mumps, measles, rubella. Two for rubella, two for measles, and one for mumps. I would think he would be approved on that one. He's pretty well covered on those diseases even if he didn't have the second dose of mumps. Many get approved on one MMR shot.

Varicella has VH so okay

Flu Has the waiver marked "not flu season", so okay.

Those first two lines cover Diptheria, Tetanus, Pertussis...adult line and baby line. He is lacking one given in the last 10 years. 1988 was his last one. So yes he is due a booster. It could get overlooked by an adjudicator in a hurry, but the darn nurse at Knightsbridge had to write on it that he has chosen to defer his required shot. That kind of calls attention to it.

Choice to make:

Find a civil surgeon who will copy all those shots from the DS-3025 plus the proof from his two others onto the immunization section of an I-693 without giving any blood test or TB skin test or medical exam. They are often hard to find. They don't understand the K1 process. $25-35 would be a reasonable fee.

Or

Send that copy of the DS-3025 with AOS and cross your fingers, knowing full well he isn't complete on that form. Hope for a miracle. If he gets an RFE, then go find your civil surgeon and get an I-693. He's got so much stuff on that form that may not bother to decipher it. An RFE is no big deal now that he is here with you.

I really appreciate your response. I wasn't sure if TB/blood test was standard with civil surgeons and was a bit surprised that another Feb K1 filer had to get those. They ended up paying about $150 for all and the place I called wanted $260 and didn't seem willing to just do the transcriptions, but I will have to call around! I would hate to risk just sending what we have now, although you are so right, now that he is here, the timeline doesn't matter as much. Will they process the AOS separately from EAD? I'm worried if we got RFE for the medical stuff, it will delay the EAD.

People have gotten their EAD and later at interview the guy says you are missing a shot. So there are many cases where EAD is approved prior to anybody going over his shot record in detail.

There is a thread that you can search for call "Civil Surgeons Who Don't Rip you Off". Put that in the advance search box (top right corner of this page to find the advanced search). People listed ones they found and somebody near you may be listed. You have to go through the whole thread.

Your K1 friend had procedures not needed. Do NOT believe a civil surgeon who says you do. Good luck. It is very frustrating.

Hi! I know you are confused with this process as I was. I am set for my interview for my AOS; I have NEVER seen a Civil Surgeon and did not receive an RFE for that; USCIS have scheduled me for an interview... I ask my lawyer about this; and he said that for K1, as long as we have filed for AOS within a year of the medical exam that we have in our home country then we are not required to see a CS; or else I should have received an RFE instead of an interview date... Hope this helps.

You can receive an RFE at the interview, or even after the interview.
