Hello.
I am applying for PIO card for my 8 month old baby at Chicago CKGS. He was born in US (2013) and has a US passport since birth. Both me and my wife were born in India. My wife was an Indian citizen from her birth in 1984 till 2011 (US citizen since 2011 by naturalization). I am still Indian citizen with US green card. I need help with questions below

1. Question # 14 (Permanent address in India) verifiable: Can I provide my address in India? Will my Indian passport which has my address listed be a valid document for them to verify?

2. Question # 23(a) Whether the applicant or his/her parents or grandparents or great grandparents held an Indian passport at any
time, if so details thereof: (enclose photocopy of relevant documents)
: I hold an Indian passport from birth until present; my wife held Indian passport from her birth (1984) till 2011 (US citizen by naturalization). Do I need to include details of both of us?

3. Question # 24(a) Whether the applicant or his/her parents or grandparents or great grandparents was born and was permanently resident in India as defined in the Government of India Act, 1935 * at any time {please see instruction (1) }. If so, following details may be indicated: : Both me and my wife were born in India. My wife was a resident of India from birth until 1999; I was a resident from birth until 2007. Do I need to include details of both of us?

4. Question # 25, Proof of residence etc.: Will my Indian passport which has my address listed be a valid document for them to verify? what documents could I provide as a proof of residence for my wife?

You can provide a valid indian address

If applying for kids, it is better to have father's or mother's information... does not hurt to provide both parents

YES

4. Question # 25, Proof of residence etc.: Will my Indian passport which has my address listed be a valid document for them to verify? what documents could I provide as a proof of residence for my wife?

After marriage, your wife has your address.... your address will be her address in India...

I don't know why you are so into "having your passport address to verify the address in India" It is good if both are same... it can be different too.

How embassy want to verify the address..... that does not matter... they need a verifiable address in India

Thanks for the response Deep2009.

I was trying to complete the "Parental Authorization for Minors" that is required for PIO for minors.

They have a question stating "do hereby authorize my child to leave United States of America, from ___ to ___"

We do not know our travel dates yet and our child would be travelling with us. What should my response be?

Would anyone please help me with questions below:

1. Who signs the CKGS order form (I am assuming both parents need to sign it)

2. What dates should I add in the "Parental authorization form for minors" for "Do hereby authorize my child to leave the United Sates of America, from ___ to ___" ? I am just applying for the PIO card now, and I am not sure of the travel dates yet.

3. I am paying $15 for the Fedex Courier Services.Do I still need to send some kind of return envelope?

4. In the "Additional Particulars Form" on the last page for declarations, they have the following question:

I am applying for a PIO card (select whichever is not applicable)
(a) As a spouse of an Indian origin person
(b) Not as a spouse of an Indian origin person

For me option (b) is applicable, but they mention "select whichever is not applicable". Does that mean I should select option (a)? They have asked the question in a very confusing manner.

5. Should I staple sets of different forms separately? Do they have any specific instructions on how to put together the package?
How should I include the money order and 4 photos (in an envelope?)
