All applications are ready to file for adjustment of status. However, we are just missing the I-94 records. I tried to enter all variations and all possible entries on the their web retrieval system https://i94.cbp.dhs.gov/I94/request.html but no luck. She was investigated by immigration officer at the airport and has a stamp that says F1 D/S along with the visa annotation number/.

Can we send I-485 and I-130 without I-94 records?

You can send a copy of her passport stamp in lieu of the I-94, but don't be surprised if they still RFE you for the I-94 itself because that's what they really like to see.

You may need to visit a CBP Deferred Inspection facility in order to get the I-94 issue corrected: http://www.cbp.gov/sites/default/files/documents/deferred_inspection_sites_052914.xls

Agree with Hypnos. Actually, the link i provided, and where you said that you had followed all of the instructions, included information about CBP deferred inspection.

I had the same problem with my stepdaughter's I-94. They had the names in the wrong order. I went to one of the secondary inspection locations (usually in or near the local international airport or sea port) and they fixed it no problem. Went home and printed out the I-94 from the website.

She is during 60 days grace period after graduating. Would going there raise red flags or open a can of warms on her?

She needs to get it corrected. It can certainly cause problems for her, resulting in red flags and cans of worms if she does nothing. Go with her and explain what happened.

We had to go CBP Deferred Inspection station to correct my stepson's I-94 the day after his arrival. It was pretty painless.
