HI Everyone,

I need help. I am a K-1 Visa holder and I am looking at the documents needed to submit the I-485 for AOS.

It says that I need a vaccine supplement form, the problem is that the US consulate back home never sent me "certified sealed" copies of the original DS-3025 (though they claimed they did). I can't get another copy because they don't keep copies. The original medical exam was in the sealed envelope that I had to surrender at the point of entry.

I know the results of my medical exam, I had every shot necessary already apart from the flu shot (which I got done).

My question is, what do I do? I called USCIS and they said they didn't have my files and was directed to Borders and Customs and spoke to a lovely gentleman who was just as confused and advised me to contact the local congressman. I actually did this, and I was told that the state department has the sealed envelope and that is why USCIS can't see it.

*sigh* I just want someone to tell me whether I should just apply for the AOS without any vaccine records (since they have the original) or if I should go about finding a civil surgeon and do a I-693 or something.

**By the way, I also contacted the physician back home and they only have a list of all the vaccines I've had done (not a copy of the DS-3025) and have arranged to have those mailed to me.

Ok, so I should either file without a vaccine record and see if they send me an RFE or just go see a civil surgeon to get my vaccine record from my doctor transcribed onto a I-693? (is that even possible if it's not from a DS-3025)?.

If you have your full vaccine record, you can go to civil surgeon to get the record transcribed onto I-693.

IMHO, If you can find civil surgeon that will do the I-693 transcriptions cheap, I will just go with it, instead of worrying about getting a RFE day in day out.

The pain of waiting is horrible -_-

Also, I found this older thread, where they talk about number of civil surgeon doctors

that will transcribe to I-693 at very nominal price. Some of those doctor contact information

may be outdated by now, but looks like it may be possible to find one that will do the

transcribing cheaply...if it becomes necessary.
