We received packet 4 today! Just to let everyone know, in the packet there was specific instructions for filling out the new I-156 form. I went ahead and did mine last week and just now had to redo it. The only real changes were that they didnt want you to fill in the country or province of issuance for the passport in section 2 of the form and they didnt want any N/A's or None's in any blank fields. We receive ours today as well. My fiance says there are 3 forms....ds156, ds156k, and ds230 part 1. Are we suppose to receive ds230 part 2?

I dont think they need the DS part 2, maybe they fill that out during their interview or something. I made a, 'things you probably wont need, but just in case they ask, you have them,' folder. Inside that's the ds part 2. Make sure she, or you, do the I-156 online, theres a barcode on it after you print it. I dont know why they even included them in the packet.

I got the vacination today, it's crowded at that place. It took me a half day to wait for only 2 shots for vacination. I feel tired now. I think that I will go to the Medical Exam in the next 2-3 days. I got packet 4 with my appointment letter and one appointment letter of the next person who has interview time after me.

I came back to the Consulate to return it them and to inform them that they missed that one in my packet. I has seen a long row of people who queued for their interview. They came really early, it's around 1 PM but I saw more than 50 persons in line to come to the US Consulate already. It means we have to come to the Consulate at least 1 hour before the interview time.

I don't have the form DS-230 part 2 in packet 4. But I forgot to ask them. The best way is to make it ready for in case they may ask for.

Now is the time for packet 4's preparation. There is one couple with the woman who has the same interview date with me. I way see her at the Consulate in the Interview day.

Quick question on the form you guys got on the Package 4.

Form DS-156, and DS-156K, and DS-230 Part 1 and 2. I thought the DS-230 part 1 and 2 already get turn in on the package 4. Now we have to resubmit it again. Also, on the DS-156 and DS-156K, this need to be fill out on the Web right? not on the actual Form they sent. This is confusing, Also on the web, the only form need to fill out is the DS-156. Can you guy clear it up for me. I just want to get all the paper form ready when i be back there in Feb. Anyone have a complete scanned package 4 please upload it here.

What about Section 2 2.Place of Issuance City? leave blank also?

So are you suppose to fill out the online DS-156 or not?

Could you post exactly what is says about the DS-156? This has me scrambling to fill out several DS-156's for her.

Hi Lurker,

You are supposed to fill out the DS-156 using their link to a form because it takes the information you fill in and generate a bar code which contains all of your information. You are supposed to prepare 2 of these forms.

The DS-156K is a fillable PDF file that can be filled in and printed out. You can do it by hand as well but it is neater if you use the PDF file. This document must be signed in front of the officer.

The DS-230 is the same way. You can also use the PDF file format to fill and print out; or you can fill it out by hand as well.

Your fiance, who stayed far from HCMC, should come to HCMC at least before one week, to take the vaccination and medical exam. She will need to take a rest 2 days before the medical exam.

For the issuing country, I selected Vietnam from the drop down menu, and put N/A in the state section. I believe that the instruction says not to leave any field blank on any form; use N/A for those fields that do not apply to your case.

Darn it. Ok...well i was going off the original poster and what they said by not putting N/A because they said the instructions said not to use N/A or None and not fill out the state or province of issuance. This is painful.
