places i lived almost 6 months since the age of 16, they just require the city, province and country or i need to put the adress?

and can this form be filled ellectronically or have i to use the one included in my packet 3 wich would be hand written??

To answer your questions. The DS-230 has no place for street address, Just City, Province, Contry in section 20 "Places lived"

Yes you can ues the fillable PDF, it is preferable to help prevent mistakes at the consulate when they enter the data into their systems.
