Filling I-765 when I had an OPT in the past

Hi guys, like I mentioned earlier, I'm a F1 student trying to adjust my status after marrying my wife (USC)... now, I have a question filing form I-765 since I had an OPT in the past when I finished my master's, prior to going back to school. In line 11, I mark Yes right? the USCIS office from which I received the letter is Mesquite, TX... is that the office that I use?? I did a quick search and I didn't find that office....it was back in 2008

Lastly, in line 16, do I still put ©(9)??

I am currently on a J-1 and adjusting based on marriage to a US citizen. Before I met my wife, I was on an F-1, for which I did also get an EAD for post completion OPT.

So on the I-765, I did mark "yes" to that question. I dug up my old paperwork for the F-1 OPT and included photocopies of it with my packet. I clearly annotated the photocopy to show that the OPT was related to my prior F-1 status, not my current J-1 status. I assumed this was the correct thing to do, though I cannot tell you for sure since my EAD has not yet been produced (my online status says "initial review").
