hi there VJ members i need help on adjustment of status anyone recently done it? my husband enter us and didn't have a copy of his vaccination record and it says there that i need to send it to file the adjustment status now i need to go to immigration doc.. thank you so much in advance

So what's your question? He doesn't have record then he needs to go to a civil surgeon to have vaccination assessment filled. Locate your civil surgeon here:

Did he have any record when he went for his medical or did he have any jabs at his medical. The original 3025 goes with the immigration paperwork he would have brought with him and normally at the medical you get a copy of it. So maybe his vaccination record is already with USCIS

Did he have any record when he went for his medical or did he have any jabs at his medical. The original 3025 goes with the immigration paperwork he would have brought with him and normally at the medical you get a copy of it. So maybe his vaccination record is already with USCIS

Of course USCIS has a copy of the DS3025. That is put into the sealed envelope that you hand over to the CBT officer at POE. But you are still required to put a copy of the DS3025 with your AOS or you will get an RFE and then need to submit an I-693 that will need to be filled out by a CS. Hope this helps.

You are not required to put a copy of your DS-3025 in with your AOS paperwork. The instructions for the I-485 state you may include it. People only put the copy to show they complied with the vaccination requirements. Some people do not have a copy of their DS-3025 and add a note instead reminding them that they had all the required vaccinations at the K-1 medical and the forms can be found in their case file that the USCIS has.
