The copy of my DS-3025 has the following ticked under Results:

- Vaccine History Incomplete

--- Applicant may be eligible for blanket waiver(s) because vaccination(s) not medically appropriate (as Indicated Above).

I have a bunch of "Not Age Appropriate" ticked: DT/DTP/DTaP; Polio-OPV/IPV; Rotavirus; Hib; Hepatitis A; Pneumococcal

And then I have Td/Tdap ticked as "Not Routinely Available". The panel physician whom I saw at my home country of Malaysia said that this specific tetanus vaccine is not available in Malaysia.

So in light of my current situation, do I need to see a civil surgeon to fill up my i-693? I'm currently living in New York and I believe the fees of seeing a doctor are really high. I'd like to avoid spending unnecessarily unless this is absolutely required.

I'd appreciate your advice, thanks a lot!

You need at least one in a series of MMR, Tdap/TD, varicella(unless you already had the chickenpox), and a flu shot(only during flu season, which currently it is not). You stated that you are for sure missing the Tdap/TD. You need to find a civil surgeon that will do just the vaccinations and not insist on a whole new medical. You need any missing vaccinations and the vaccinations transcribed onto an I-693. That's very helpful, thanks a lot! :) Yeah I was afraid of being ripped off by civil surgeons.

I tried to figure out how to post a new topic in this Malaysia portal, but I don't see how, so I guess I'll just post my question in this thread since it is on topic.

We are currently waiting on our NOA2 for a K1 visa. We want to be as prepared as possible for when she does receive notification that she needs to have her medical exam (which she will do in Penang). She is 50 years old and has no idea how or where to get her vaccination records from when she was a child. She is certain she had MMR and Chickenpox vaccinations -- but no documentation. Is this going to be a problem for her when she goes to Penang for the required medical exam? She lived in a small city when she was a child and has no idea how to obtain such old records -- if there even are any. What can we do in advance so she can prepare?
