Just for fullness of information, can someone advise OP if he still need to undergo full medical as most of the replies so far talks about only losing the fee paid for the K-1.

The old medical is still valid (for one year), just like for K-1 AOS, even though the type of AOS will be different.

Also for fulness of information, a lot of K-1 cases get their interviews waived for AOS, but I guess OP is likely to have one now since it's regular spousal AOS?

Yes, joint interviews are mandatory for a I-130/I-485 concurrent AOS.

You can also call the IRS and have them fax you your tax returns, it's a little tedious by phone but you will still have them by the end of the day. Everything else is pretty straight forward,

Yes, and besides calling, you can also go in person to an IRS office and get them the same day too.

Because they didn't get married within the validity of the K1 visa and that clearly complicates their situation.

The affidavit of support you submitted with the K1 isn't the same as the one you're going to need for the AOS. The one you needed for the K1 process was the I 134, now you need the I 864 with the AOS. Also, if your financial situation changed meaning you're not making enough, 125% of the poverty level, then you would need a joint sponsor.

Also if you're financial situation is what affected your being able to get married within the 90 days, it can help you explain why failed to meet the 90 day marriage requirement should it come up during the AOS process.

A marriage license is not expensive. Filing a lot of the forms can be expensive. However, going to the courthouse to get married is not an expensive endeavor. Having the big white dream wedding? Yeah, that's expensive. But signing some forms for a civil ceremony at City Hall is not.
