I am about to file for AOS and have seen conflicting posts so am looking for clarification on this issue.

When I completed my medical at Knightsbridge Doctors in London for my K1 visa, they completed ALL my required vaccinations so I wouldn't have to worry about it when I got to the states. The nurse gave me a list of what I had but told me i didn't need it as it would be included in my medical sent straight to the embassy and kept as part of my big brown envelope. I am now filling in the forms for AOS and notice that an i693(or supplemental 3025) is required as proof of these vaccinations?

I don't have any official proof of this but as I said they should have all the info already, would it be sufficient to point out in my cover letter that the i693 is not included as they are already in possession of the information?

You still need a civil surgeon to transcribe your vaccination record (3025) and you are to submit a sealed envelop from that civil surgeon. There's no shortcut in that part. Sometimes the civil surgeon may require more vaccine due to seasonal requirement.

You still need a civil surgeon to transcribe your vaccination record (3025) and you are to submit a sealed envelop from that civil surgeon. There's no shortcut in that part. Sometimes the civil surgeon may require more vaccine due to seasonal requirement.

Op said they completed all of the vaccinations prior to arriving in the U.S. And that was included in their medical which was received at POE.

I-693, Medical Examination of Aliens Seeking Adjustment of Status.I-693, Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. If you are missing any shots from your overseas medical a I-693 vaccination transcription is required to be completed by seeing a Civil Surgeon to have certain portions of the I-693 completed and have the Civil Surgeon certify the form (sealing the original in an envelope and giving you a copy).

Op you don't need to dee a civil surgeon.

I found this form a post by Nich Nick who I have known to be fairly accurate in these matters, which I am going to include in my cover letter. I think this should suffice, as long as they haven't lost my original documents submitted at POE.

"Please note that I am adjusting status from a K1 visa. I had a medical exam on MARCH 10 2014 at Knightsbridge Clinic in London, United Kingdom. Because 12 months have not passed since that exam, I am not required to have another medical exam. Please see my medical files which I turned over at POE and include a properly completed DS-3025 to show I have complied with all vaccination requirements."

Any thoughts?

No, you don't. Many are approved with or without interview without ever seeing a civil surgeon for an I-693. Also, if the seasonal flu shot was not required at the time of the K-1 medical, then you still do not have to go out and see a civil surgeon for it just because it happens to be flu season when you file for AOS. You only need one shot in a series and what was required at the time of the medical when the DS-3025 was filled out by the panel physician.
