Hi everyone

we have looked over the example forms, but we are still confused.

It says on the form I-131 to skip to part 7 after filling out part 3, but it seems there are two questions that are not answered on the example form that seem important (and are STILL included in part 3)

That is Question 4a:

Have you ever before been issued a reentry permit or Refugee Travel Document? (If "Yes" give the following information for the last document issued to you)

and Question 5:

Where do you want this travel document sent? (check one).

5. To the U.S. address shown in Part 1 (2.a through 2.i.) of this form.

These questions are still part of PART 3, but they are not checked in the example forms. Should we fill in/check these boxes?

You would do well to avoid overthinking the form. The poor design could drive you crazy. The form is designed two columns per page (kinda). The pertinent directions come into play at the end of the first column at the end of question 3.b., Skip the rest of the questions starting at the top of the second column (as directed). :unsure:

You can check No for Part 3 #4a if you wish, but #5 definitely comes after this statement on the form and is something you skip > If you are applying for a non-DACA related Advance Parole Document, skip to Part 7
