Lol this is completely false. You CAN attend medical without an interview letter and many people do so across various immigrant visa categories. When I was immigrating I only notified the hospital what I am booking an appointment for, I was never required to show an IL, neither they should be privy to my personal affairs and letters.

And also people in F2B quite often experience delays receiving not only ILs but also WL. There's quite "a few" threads on the matter.

You are now following me? Almost all hospitals in the western world will require your IL

to do the medical.

I think that user might be a troll. Doesn't volunteer personal info but likes to state what "thousands" have gone through. As if "thousands" was one potent voice. 

 

In any case, many people in this thread have voiced their individual experiences and none are straightforward. Each case and country have their own pace.

You can keep your status as 'single' until your VISA is granted. You can sign marital status with her after your visa take place in your passport only.
During prepare DS-260 to NVC in future you can also add you children name with your personal information. 
However for your spouse, you can file I-130 for her after you relocate to the US. It would take around 2 years waiting in category F2A.

I have seen many people in your situation. I will advise you to update your marital status. If you retain "single" as your marital status, you lied although you might still migrate with your child. However, your wife will never migrate. And if it's eventually discovered that you lied under oath that you were single at the time of migrating, even after becoming a citizen, the citizenship will be withdrawn. During the interview, you will be asked to take an oath that you were still single. I know because I came with F2B visa.
