if your DS-3025 vaccination record(you should have a copy)is filled out correctly you are good, but,if not, you will need to go to a civil surgeon for him to complete a I-693 supplemental medical.

yes it is a good idea even tho U.S.C.I.S. will have the original from inside the sealed envelope.

im not a computer guy , don't know how to give you the link but if you do a DS-3025 search here you will find a copy of a correctly completed DS-3025 to check yours against.

No, you only need to include your i693 if you did not get all your shots at the medical exam. If the medical exam was done within the last 12 months, the civil surgeon only needs to do the vaccinations part of the i693

If you file for AOS within one year of your medical you do not need another medical. Reading page 4 section 7c of the I-485 instructions you only need to send in a copy of the DS-3025. (I suggest you print a copy of page 4 and highlight section 7c then staple it to your copy of the DS-3025 as some adjudicators at USCIS are not the brightest. )
